MGB is growing! Our GRACE Team is hiring an experienced Nurse Practitioner for our growing regional team, Geriatric Resources for Assessment and Care for Elders
Hybrid position requires 3-4x weekly onsite, travel to patient homes
South Region - Boston, South Weymouth & Foxborough Patriot Place. Patient homes reside within 25 miles of the practices, some practices are in Boston.

This is an exciting opportunity to join the Mass General Brigham Population Health department as a Nurse Practitioner within the GRACE (Geriatric Resources for Assessment and Care of Elders) program. The GRACE Nurse Practitioner will provide home-based geriatric assessment and care management for older adults aligned with Mass General Brigham’s Primary Care Providers.
This role is designed to proactively support vulnerable older adults, ensuring comprehensive, coordinated care that aligns with their goals and preferences.
As part of this initiative, the GRACE Nurse Practitioner will play a key role in:
1) Conducting in-home geriatric assessments in collaboration with a GRACE Social Worker.
2) Developing and implementing individualized care plans using GRACE care protocols.
3) Engaging patients and caregivers in health education, chronic disease management, and goals-of-care discussions.
4) Coordinating care with primary care providers and the GRACE interdisciplinary team to optimize outcomes and reduce unnecessary hospital utilization.
This role is an essential part of Mass General Brigham’s commitment to improving care delivery for high-risk older adults through proactive, patient-centered management. The GRACE Nurse Practitioner will build strong, lasting relationships with patients—offering culturally responsive care, continuity, and education to help manage complex health conditions and promote maximum functioning.
We are seeking a full-time, 40-hour Nurse Practitioner with weekly schedules consisting of 36 hours of clinical, patient-facing time, and 4 hours of administrative time.
This will be a hybrid role with the expectation that the Nurse Practitioner will conduct home visits in the South Region.

Key components of GRACE Team Care include:
- In-home assessments by a nurse practitioner and social worker team.
- Individualized care plans developed using GRACE protocols.
- Collaborative care planning with primary care physicians.
- Ongoing home-based care management and coordination.
- Weekly interdisciplinary team meetings including a geriatrician, pharmacist, and mental health liaison.
As an independent practitioner, the GRACE Nurse Practitioner exercises clinical judgment and autonomy in evaluating medical and functional statuses, planning treatments, and applying nursing ethics to complex geriatric care scenarios.
What You'll Bring
- Master's Degree Nursing required

- Nurse Practitioner [NP] / Advanced Practice Registered Nurse [APRN] [MA State License)
- Experience as a nurse practitioner, 3-5 years highly preferred
- Valid Driver's license and vehicle, commuting to patient homes is required

Working Model Requirements
- M-F Eastern Business hours
- South Region: must be accessible 4-5x weekly to South Weymouth Main St, and Foxborough Patriot Place practices
- Must travel to patient homes who are within 45 miles of MGB practices. If patient goes into a practice, within distance, that is possible as well, NP must be flexible for business needs and training as needed, or meetings planned onsite at MGH or Somerville.
- 1 remote day per week is likely, based on business needs, must have quiet, compliant work space for remote work
